New Contracting Procedures for
Agency Appointments
Please alert your licensing staff to this requirement.
To streamline and ensure accuracy of commission payments, EquiTrust will revise contracting procedures for agency appointments. Effective January 1, 2016, agency contracting must include the attached Agency Business Entity Certification Form (Form ET-AGT-ENTITY-3503), as well as a copy of a corporate resolution.
In addition, certain requests for changes to currently contracted agencies will require this form. These include, but are not limited to, changes with respect to:
• Authorized signors
• Agency banking information
• Address changes
