the Re-filed Certainty Select Contract
Effective February 5, a re-filed version of Certainty Select will be offered in six states: Indiana, Minnesota, Oregon, Texas, Utah and Washington. The new contract is form series ICC13-ET-MYG-2000(07-13), and has variations from the original Certainty Select product filing.
For these six states, the following variations exist:
• Surrender Charges: 9%, 8%, 7%, 6.5%, 5.5%, 4.5%, 3.5%, 2.5%,
1.5%, 0.5% (Surrender charge period matches guarantee period)
• Base Contract only; Optional Rider is not offered
Application Variation
Certainty Select applicants in these six states must use application form ICC-ANN-APP(05-13), available now on the EquiTrust website. Earlier today you were notified of the new generic annuity application form and transition rules. The grace period referenced for the generic application does not apply to Certainty Select sales in these six states. New Disclosure Statement
In these six states only, Certainty Select applicants must use a state specific disclosure form dated 2-14, which is available now on the website.
Revised Product Materials
Certainty Select client brochure and agent guide are the same for all states, with variations referenced accordingly. Revised materials, dated 2-14, are available now in PDF form on the website, and in printed form at the "Order Supplies" link.
