This version is required for all business written in the state of Florida. It is a great tool to capture pertinent information about the Owner/Annuitant and Beneficiary. In addition, you no longer need to complete a separate W-9 as this information is now included in the Application. Effective immediately, the new application for Florida will be form #12 A-2000.1 dated 1.2.13. Click here for a copy of the new form. Please discontinue use of Annuity Application form #2000-FL (Rev) (1/1/10). This version will no longer be accepted after March 29, 2013.
