Choice Series Application deadlines - Applications with cash that are received in good order in our service center by May 15, 2015 will be issued as applied for.
- Non-cash applications (e.g. 1035 exchanges, IRA rollovers) that are received in good order in our service center by May 15, 2015 will be issued as applied for, provided funds are received no later than June 26, 2015.
- Faxed applications will not be accepted.
