Effective Friday, October 31, 2014.

 

Current rates are applied to business received on or before
Thursday, October 30th, subject to a 60-day rate hold.

Please note:  EquiTrust will only accept faxed applications on Thursday, October 30th until 5:00 PM (Central). The ORIGINAL application paperwork must be received by Friday, October 31st.
