EquiTrust Annuity Forms Revised
Available For Use Now; Required on 1/1/13 Effective immediately, all Annuity Product Disclosures, Rider Disclosures, Financial Needs Analysis and Disclosure & Comparison of Products forms are revised. The revised forms will be required beginning 1/1/13. New forms are available on the Agent Website now and may be used immediately.
Note: Product / Rider Disclosures are revised for ALL ANNUITY PRODUCTS and the Income For Life rider.
Also revised:
• Financial Needs Analysis form (ET-2506)
• Disclosure & Comparison of Products form (required for replacements)
To assist in completing the Disclosure & Comparison of Products forms accurately, a product-specific version is now available for each index annuity product.
